The following requirements must be met before any student-athlete will be cleared to tryout, practice or participate in any/all aspects of ASHS Athletics. It is strongly encouraged to complete all requirements over the course of the summer months/prior to the start of the school year.
ATS Medical Records Account
Every student-athlete must have an ATS account. The same account will be used for the duration of the student-athlete’s athletic career at ASHS. This account will require yearly and/or seasonal updates.
Please create or update the account.
Physical Examination Forms (yearly “Sports Physical”), Part I and Part II
This MUST be completed on the ASHS Physical Examination Forms. Part I (questionnaire) must be completed and signed by the student-athlete and parent/guardian, and reviewed by the physician, physician’s assistant or nurse practitioner. Part II (physical examination) must be completed and signed by a physician, physician’s assistant or nurse practitioner. A physical is valid for 365 days, as dated by the medical provider. We prefer all student-athletes be on a June/July to June/July schedule, so as the physical remains valid for the duration of the school year. Special circumstances do arise. It is preferred that all physicals are turned in for the 2018-2019 school year by July 25. At the latest, physicals are due by August 1 (fall sports), Oct 15 (winter sports), and Feb 1 (spring sports).
If the student-athlete is an incoming freshmen or transfer,
there are additional forms that are required for the School Nurse. These would include the Immunization Record Form (mandatory) and the Maryland State School Medication Administration Authorization Form (as desired). These are not associated with Athletics and should be returned to the School Nurse. The Physical Examination Forms Parts I and II can be found here.
“Spring” Athletic Participation for rising 8th graders/incoming freshmen – the most recent “well child/yearly physical examination” must be submitted to the Athletic Training Staff before any participation. In addition, they must have an ATS account.
Parent & Student Yearly Athletic Waiver and Agreement Forms
The student-athlete and their parent/guardian are required to read the forms in their entirety. Parents/Guardians and student-athlete must complete the online electronic forms together and submit. It is preferred that these forms are completed for the 2018-2019 school year by July 25. At the latest, forms are due by August 1 (fall sports), Oct 15 (winter sports), and Feb 1(spring sports).

Neurocognitive Baseline Testing, ImPACT
This is a computer-based testing tool used in the management of mild traumatic brain injuries (mTBI), more commonly known as concussions. The student-athlete will take a baseline test, so that in the event they sustain a concussion or an injury with a mechanism that suggests a concussion, we may be able to evaluate the severity of the injury and the progress of their recovery.
This is mandatory
for the freshmen and junior class student-athletes, any student new to athletics or transfer students
. Testing is available for all student-athletes, however not required for boys or girls cross country, girls tennis, boys or girls swimming, boys or girls indoor track and field, boys or girls outdoor track and field, boys tennis, and boys and girls golf.
ImPACT testing is completed on your home computer, under parental supervision. It is preferred that all testing is completed for the 2018-2019 school year by July 25. At the latest, it must be completed by August 1 (fall sports), Oct 15 (winter sports), and Feb 1 (spring sports).
