• 2026-2027 Fall Sports Tryout Information

  Any student who wishes to participate in athletics at Spalding must have the below information completed annually in their Augusts Schools Health Account prior to August 1st.

   

  1. Annual physical completed by parent & physician on Spalding Physical Form

  2.  Immunization Form (required for incoming freshmen & transfers only)
  3.  Medication Authorization Form (if applicable)

  4.  Asthma, Allergy, Diabetes, Seizure plan (if applicable)

   

  All forms must be uploaded directly to the August Schools Health Account which can be accessed through your parent portal - paper or emailed copies will not be accepted.  Required Health Forms Uploading Instructions 

  
  Tryout dates, times, and locations for each fall sport will be posted below as that information becomes available.
