Archbishop Spalding High School has transitioned to a new Student Medical Record software to streamline your child’s required medical and school forms. Magnus Health Student Medical Record (SMR) is a web-based system that allows parents continuous access to your child’s health record as well as the ability to make updates when needed. 
 
All medical forms and information will be uploaded directly to the Magnus Health Account SMR – no paper copies should be mailed in to the school. Physical forms must be completed on Archbishop Spalding’s paperwork which can also be found on the Magnus Health Account under the ‘Annual Physical Exam Form’ section. Athletes must upload a current physical to the Magnus Health Account annually in order to participate in Spalding athletics. Please plan to upload your physical and any medical documents to your child’s Magnus by August 20th, 2021. 
 
To access your Magnus Health Account: please log in to Spalding’s website and click on the button titled “Magnus Health Portal” on your Blackbaud Resource Board. 
 
SYMPTOM SCREENING
Archbishop Spalding is pursuing an aggressive mitigation strategy to keep our community as safe as possible for students, staff and our families. An additional step Archbishop Spalding is taking is to require an at-home symptom check to be performed before each student-athlete may report to school, practices, workouts and/or games each day. The Magnus Health system app includes a symptom screening survey that every student-athlete must complete prior to being on campus. 
 
To access your Magnus Health App: please login to your Magnus Health Account as described above. Hover over your name in the top left corner of your account and click “change credentials”. Create a username/password – this is what you will use to login to the app. Download the Magnus Mobile V2 app to your phone. Enter the credentials you just created. Locate the “COVID19” button and click on it to complete the symptom survey.  
 
POSITIVE COVID-19 CASES IN ATHLETES AND RELEASE FORM
All student-athletes who have had a positive exposure or test positive for COVID-19 must inform both the school nurse, Maura Martin, and head athletic trainer, Kelley Crowe. All positive/presumptive positive cases of COVID-19 in student-athletes require an athletics-specific return to play form in addition to the school release form (link located on this page). This form should be completed by your child’s primary care physician after completion of quarantine. If your physician requires further cardiac workup, please reach out to Kelley Crowe for information about scheduling with the school’s affiliated cardiologist.
